The required Program of Study form is available from the UND Graduate School website and can be downloaded from this address: http://www.und.edu/dept/grad/html/mastersforms.htm.

It is the responsibility of the student, in conjunction with his/her Faculty Advisory Committee or major advisor, to obtain the necessary signatures and submit the completed form to the UND Graduate School.
